Abstract

Background Inflammatory myopathies (IM) are a group of rare inflammatory muscle diseases. Reduced strength and endurance are the main symptoms. Although various studies show positive effects of exercise, there are no disease-specific training guidelines.Objective Providing an overview of exercise studies in adult patients with IM to derive evidence-based, disease-specific training guidelines.Method At first a systematic literature review was performed. Based on the results, training recommendations were then developed. Long-lasting clinical experience of specialist physiotherapists and the training guidelines of the American College of Sports Medicine were also incorporated.Results The systematic literature review included 24 studies with a total of 151 IM patients. All studies showed positive effects for strength and/or endurance training. No adverse events were reported. The training guidelines consist of an introduction part on general training principles and two further parts on strength and endurance training for each disease activity (early-active, active, stable).Conclusions Although training protocols related to myositis are varying, all studies consistently indicate that the training is safe and effective. The training recommendations allow therapists a quick overview of evidence-based training guidelines. The guidelines should be applied, reviewed, and refined in further studies.
