Abstract

To illustrate the pathology seen at the STI-VulvalProctology
outpatient department 6 short cases are presented.
The selection was based on; (case 1) because it
needs to be highlighted that there is a suspicion that the
correct diagnosis often is missed; (casus 2 en 3) because
it is an interesting and rare diagnosis; or (case 4) to illustrate
the effect of a specific treatment. Case 5 and 6 gave
us reason to relook at the classification of vulvar intraepithelial
neoplasia (VIN).
Case 1 is a patient with vulvar vestibular papillomatosis;
a benign non-HPV related condition in which there is
hypertrophy of physiological glands located at the vaginal
vestibulum. This skin condition has a typical aspect comparable
with pearly penile papules in the male patient,
which easily can be misinterpreted as condylomata acuminata.
Case 2 deals with a patient diagnosed with a rare diagnosis
of calcinosis cutis of the prepuce (recently accepted
for publication. (‘Rare wratten’. Voeten LM et al.).1
Case 3 is a patient with a diagnosis of porokeratosis ptychotropica.
A similar case has been published by Lucker
et al.2
Case 4 is a HIV seropositive patient with persistent
anogenital HSV type 2 infection despite systemic HSV
antiviral therapy who also was successfully treated with
imiquimod locally.
Case 5 en 6 are patients diagnosed with VIN III hr-HPV,
also successfully treated with imiquimod locally. This
subject is given more attention because the VIN classification
and the clinico-pathological correlation is often not
clear.
