Abstract
No patient with type 2 diabetes is the same. That is why it is important that care is tailored to their personal needs, preferences and possibilities. This dissertation focuses on the development of tools, in the form of "patient profiles”, which stimulate individual-oriented customization in diabetes care. Among other things, this study showed that patients with type 2 diabetes can be subdivided into subgroups based on their future blood sugar levels with a quick, easy and accurate tool. Based on this classification, patient care can be adjusted to make customized care possible.
