Abstract
SUMMARY
Various initiatives exist aimed at promoting physical activity, initiated by healthcare organizations,
municipalities, and sports organizations. A physical activity journey describes the different opportunities
for physical activity – from therapeutic physical activity within healthcare settings to independent
physical activity in daily life – and the pathways people can take. However, these initiatives are often not
well connected, leaving (ex-)patients uncertain about where to go after their treatment. This makes it
difficult for them to stay active and limits the impact of existing initiatives. The physical activity journey
project aims to better integrate successful physical activity interventions through the development of a
dedicated toolbox. This toolbox will provide strategies for designing, implementing, and securing
sustainable physical activity journeys, both within and beyond the consortium (e.g., a step-by-step guide
for implementation and long-term integration). These strategies are currently being identified through
literature reviews, semi-structured interviews, and focus group discussions with relevant stakeholders.
The toolbox will be evaluated and further optimized through action research conducted in four different
regions. To ensure that the acquired knowledge is effectively translated into practice, the project places
strong emphasis from the outset on establishing an effective learning network across several domains,
with a strong emphasis on knowledge transfer, implementation, and long-term integration.
