Adviezen van POP-poli’s bij ADHD en zwanger­schap; een steekproef onder psychiaters

Achtergrond
Over medicatiegebruik bij ADHD tijdens zwangerschap en borstvoeding ontbreken eenduidige richtlijnen. Dat is een gemis, want ADHD is prevalent, ook bij vrouwen in de vruchtbare leeftijd.

Doel
Beschrijven van benaderingswijzen in richtlijnen, literatuur en best practices van poliklinieken psychiatrie, obstetrie en pediatrie (POP-poli’s) wat betreft zwangerschap en ADHD.

Methode
We deden literatuuronderzoek en vroegen psychiaters van 12 verschillende POP-poli’s naar hun adviezen aan patiënten met ADHD in de zwangerschap en kraamtijd. We verzamelden gegevens over gebruik van medicatie, comorbiditeit, best practices en non-medicamenteuze adviezen.

Resultaten
Stimulantiagebruik werd tijdens zwangerschap overwegend ontraden. Bupropion kon tijdelijk een alternatief voor stimulantia zijn, evenals non-medicamenteuze interventies, antidepressiva of antipsychotica. Borstvoeding werd bij gebruik van stimulantia of bupropion doorgaans ontraden. POP-poli’s maken gebruik van gezamenlijke besluitvorming.

Conclusie
De adviezen die vrouwen met ADHD krijgen over medicatiegebruik rondom zwangerschap verschillen per POP-poli. De beschrijving van afwegingen en adviezen van POP-psychiaters biedt illustratieve inzichten in alternatieven voor stimulantia. Gewenst zijn zorgvuldige monitoring van en vervolgonderzoek naar effecten van en alternatieven voor stimulantia rondom zwangerschap en borstvoeding, evenals het actualiseren van richtlijnen.

How to advise an ADHD patient about pregnancy? Exploring hospital psychiatrists’ practices

Background
Currently, consensus on the use of medication for ADHD during pregnancy and lactation is lacking, complicating treatment decisions for fertile patients with ADHD.

Aim
To describe approaches present in guidelines, literature, and local practices of hospital psychiatrists concerning counseling ADHD-patients on pregnancy.

Method
From March to September 2024, a literature search was complemented by a questionnaire. Twelve hospital psychiatrists were questioned about how they advised patients with ADHD about pregnancy. We collected information on the use of medication, comorbidity, best practices and non-pharmacotherapeutic interventions.

Results
Most hospital psychiatrists advised against stimulants during pregnancy. Alternatives were bupropion, non-medicinal interventions, antidepressants and antipsychotics. Most hospital psychiatrists advised against the use of stimulants or bupropion during lactation. Some hospitals used shared-decision making.

Conclusion
The advice given to women with ADHD about pregnancy differs between psychiatrists. The description of their practices and considerations provided valuable insights for psychiatrists. Further research should follow on (non)medicinal alternatives for stimulants and the development of multidisciplinary guidelines.
